Tissue transfer is used for complex composite tissue defects in previously treated regions, in particular after malignant disease treatment. The increasing incidence of skin cancer in the general population has increased the number of patients with massive tumors requiring the reconstructive surgeon's expertise in free flap. A variety of recent developments in the field using free tissue transfer for orbitomaxillary and scalp reconstruction, including maxillary reconstruction, virtual surgical preparation in head and neck reconstruction, and scalp reconstruction, are discussed here. Advanced computer algorithms allow for time- and cost-saving preparation of these procedures. Free tissue transfer is a reconstructive procedure that is frequently at the top of the reconstructive scale, and is the reconstructive procedure of preference in certain cases. The ability to harvest composite tissue which matches the tissue defect in composition , surface area and volume makes the transfer of free tissue a versatile method.
