Ileal J-pouch construction is a difficult and technically challenging operation, providing significant improvements in quality of life to patients suffering Ulcerative Colitis and FAP (Familial Adenomatous polyposis). Ileal pouch construction can be approached as a 2- or 3-stage procedure. We have demonstrated a laparoscopic approach to ileal pouch construction in a 3-stage procedure, with accompanying video to aid in the education of training surgeons. Specifically, we demonstrate appropriate port placement sites; ileostomy reversal; rectal stump mobilisation and securement; laparoscopic proctocolectomy with attention towards the pelvic dissection, and to key anatomical planes and potential sources of complications. We demonstrate the importance of utilisation of small bowel lengthening procedures and our approach to J-pouch construction. We focus on the technical aspects of pouch construction (including length, favoured equipment, orientation checks), and provide tips to the training surgeon regarding mesenteric length and ensuring mesenteric twisting does not compromise the pouch. This ileal J-pouch construction video provides guidance to the training surgeon regarding a technically challenging and life changing procedure for a patient group suffering debilitating disease.
