Abstract :

When we are going to do restorative and prosthetic treatments for both aesthetic and functional needs, sometimes we found some difficulties. For example, because there is not enough space to make the prosthetics or the teeth we are going to restore are in an unfavorable position. In other cases, even though we have been able to restore damaged teeth or replace missing teeth, sometimes the results of the restorative or prosthetic work that we do cannot last long for various reasons. Anterior tooth restorations or veneers can break or fall off easily, and dentures become unstable after a while, so dentists sometimes have to redo the restorations or prosthetics that they have made. Problems like this arise for several reasons, including unbalanced occlusion, jaw movement, unideal function, or even due to vertical dimension problems. Orthodontic treatments are solutions that have to be done before performing prosthetic or restorative procedures. Teeth with malposition that interfere with the prosthetic design to be made must be corrected first. Low or high vertical dimensions must be corrected if our restorations and prosthetics are to be good and stable in the long term. However, sometimes full orthodontic treatment cannot be done. It can be because of time constraints, costs, or patients who refuse full orthodontic treatment for various reasons. Therefore, alternative treatments are needed so that prosthetic and restoration treatments can run well. Segmental orthodontics offers an alternative treatment for correcting these tooth migrations using a limited number of teeth and skeletal anchorage. Dentists do not need to perform full orthodontic treatments; they can simply focus on mispositioned teeth movements that can help prosthetic and restorative treatments. We must keep our distance from conceptual dogma and be open to new information that contributes to performing orthodontic treatment as effectively as possible. Various segmental orthodontic treatment techniques will be discussed in this lecture, starting from theories, case reports from various references, as well as patients that I have worked on myself. The discussion will also cover conventional techniques with Temporary Anchorage Device (TAD) mini screw implants, self-ligating and conventional brackets, and segmental orthodontic treatment using digital technology with clear aligners.
