Abstract :

Palliative patients have at least one and often multiple end stage chronic or serious illness/s. Due to these patients’ complexity and comorbidities often Palliative patients see multiple providers. Patients that see multiple providers are at higher risk for miscommunication, overtreatment, and poor coordination of care amongst their providers. Effective provider to provider communication is vital as it is related with favorable health outcomes such as increased patients’ satisfaction, decreased overutilization/overtreatment, compliance to plan of care, and overall health status. Palliative care providers are playing a key role in bridging this gap. Key words: Palliative Care, Overtreatment, Overutilization, Coordination of Care, Miscommunication.

Biography :

Shannon Shepherd, MSN, FNP-BC is board certified as a Family Nurse Practitioner with over 27 years’ experience in the medical field. She has worked and specialized in community-based Palliative care the past 6 years. Shannon has helped to develop 2 home based palliative care/supportive care programs in the Salt Lake area over the past 6 years and accepted the position as the National Clinical Director of Palliative Care for BristolAIM in February 2021. She was published and recognized for her contributions in developing Huntsman Cancer Institutes “Huntsman at Home” program in the ASCO Journal. Shannon absolutely loves and is passionate about hospice and palliative care.
